When talking about the stomach’s ability to descend can this include the treatment of hot flashes – which is heat
rising up or headaches from liver yang rising – which is yang rising up and not going down, or reflux, or anxiety –
with the feeling that it is rising up from the stomach? I am trying to grasp to what extent stomach can treat
ascending issues, is it only as related to digestion or does it extend further? ALso – can one tonify the stomach to
stimulate its descending action even if one is not dry on the inside and wet on the outside?
A similar question applies to the ascending action of the lung. Can lung be used anytime we want to lift up
(including energy) or does the patient have to be obese with dry skin?

Just noticing that you flipped the types for lung and stomach:
Tonify the Stomach to treat Lung excess which presents as dry on the outside and wet on the inside- the obese
banker with dry skin.
Tonify the Lung to treat Stomach excess which presents as oily skin on a thin person.

HI, according to what Toby taught and what is written in the article he wrote for the JOurnal of CHinese MEdicine,
Stomach is dry on the inside and wet on the outside and lung is wet on the inside and dry on the outside.

When communicating about Sa’am we all need to be careful about distinguishing the presentation of excess and

This is correct if you are meaning that Stomach excess is dry inside/wet outside and Lung excess is wet inside/dry
outside.

Here it seems you are suggesting that you would want to tonify Lung on a patient who is obese with dry skin. This
is not correct. When Lung excess presents (wet inside + dry outside), we tonify the counterbalancer Stomach to dry
the inside and moisten the outside.
As to your initial question, I have seen Toby choose the descending action of St+ in a patient with hypertension
where his top 2 treatment choices were St and SI. I have also heard Toby mention that the lifting action of the Lung
can be used in the absence of a strong Stomach excess presentation if the sinking symptom is the strongest aspect
of the presentation and there are no other gross imbalances present that need to be addressed first.
For me, keeping your focus on the big picture, rather than chasing symptoms, often seems to be the best way to go
with Sa’am. Of course, until it isn’t :).

Yes.
