Ulcerative Colitis and Infertility

Female age 34, petite, dull complexion.
Abdomen is not too soft or too hard, not cold. Tenderness throughout lower abdomen always.
CC: infertility, TTC since September; Hx: ectopic year ago May, fallopian tube removed.
2ndary complaint: abdominal discomfort that wakes her early every morning around 5:30ish (always during LI
time). Wants to go back to sleep a couple hours otherwise doesn’t feel good rest of day.
Diagnosed with UC as a child, had a flare that lasted from March through early/mid June, with bleeding and rectal
pain and stools even more loose and more mucus.
Recently flared has subsided but still is woken by discomfort in lower abdomen.
Cycles usually 24 days length, severe dysmenorrhea – pain days 1-4. normal bleeding amount, not clotty. Guts feel
worse w/premenstrual/menstruation. Guts feel best mid cycle.
Pulses: weak overall. wiry R chi(blood stasis in LJ), mid level wiry L guan
Tongue: has always (at least 6 years I’ve known her) been smallish with very red edges and tip.
Appearance and Demeanor: complexion is sallow lack luster, demeanor is kind, sensitive to the needles, polite.
Likes taking her herbs, they help w/anxiety. Is a therapist. Loyal. Introverted, quiet/subdued.
Respiratory: has had a constriction/SOB since mid-March. Been better past couple weeks since UC flare has
subsided. I attributed it to a LI LU Metal issue.
Eyes brightness +4
Heat/Cold: feels mostly cold, feet are cold, hands don’t feel cold.
Damp/Dry: stools always loose sometimes with mucus and/or blood. Frequency usually 1-2 times daily. Skin is
normal. Sometimes dull colored acne w/cycle.
Did KD+ treatment on day 15, 1 day post ovulation, then got period on day 24.

HI @ajrieselac,
Welcome to the Saam online community! Thanks so much for the case.
I would be happy to look at it more closely with you but ask that you do more of your own Saam analysis first. You
will find the tools for analysis here Case Study Format23/01/2024, 12:03Ulcerative Colitis and Infertility – Sa’am Clinical Questions – Qiological Community
https://forum.qiological.com/t/ulcerative-colitis-and-infertility/1111/print2/3
Your formatting is already pretty good. In your analysis please grade the signs and symptoms on a scale of +1 to
+10. As well list all of the channels and assign all of the signs and symptoms to the channels they could possibly be
associated with. Feel free to put a ? if you are unsure.
My first look doesn’t see any clear signs of SI excess, beyond infertility. There can be a whole lot more involved in
infertility than just kidney, if we look with Saam eyes or through other TEAM lenses. I do see some significant
signs of possibe Kidney excess- ie severe dysmennorhea, pms. Some information about her Kidney through Saam
eyes would help us see the SI/K balance better- how symmetrical and into her appearance is she? What do her
medial heels look like? How many varicosities?
As well, Toby says that he rarely uses Kidney as a first treatment as he finds it more helpful to clean up other areas
first.

Learning a new system is a huge undertaking, especially when it asks you to see and think in a particular way and
sometimes gives different significance to terms you have already been using.
Even if your heart is giving Saam a big YES, it is going to take some time to get the hang of it. For most of us, at
least one lifetime to really get the hang of it.

Here’s an addendum:
Resources: patient watches her finances as far as spending w/treatments and supplements.
Energy: overall low +7
Symmetry/beauty – average to below average? so maybe +4? not sure how to rate.
lackluster complexion +9
Doesn’t wear bra to treatments, and breasts look saggy +5.
Overall impression is a tired dullness – quiet, introverted. +8
Sensitivity to pain +7
Menstrual pain +723/01/2024, 12:03Ulcerative Colitis and Infertility – Sa’am Clinical Questions – Qiological Community

colitis loose stool +8
rectal pain “prickly” comes and goes w/flares +9
lower abdominal discomfort “tender and heavy” waking her from sleep around 5-6am +10
tends to anxiety especially from discomfort/pain +8
low libido +7
Low mood sometimes +6
No varcosities I’ve noticed. Skin overall smooth not rough. Medial heel condition unnoticed, i can check next visit.
Overall, sensing an Liver Excess, and a KD Excess.
Plan: #1 SUpplement SI to move blood and alleviate pain.
#2 Supplement SJ to add brightness as she seems a bit “cloudy-weather”.
#3 Then because she’s blood deficient (weak pulses, chronic bleeding, dull complexion, cold) – see if
supplementing Liver comes up as a good treatment after doing 1&2
#4 supplementing LU to address low energy in terms of lower resources, but also to address chronically loose
mucusy stool. (not confident on #4 as there’s no convex/concave, and don’t know what she drives even after all
these years!)

It is really helpful to list all of the channels and consider how her signs and symptoms relate to these channels. You
may be surprised at what you see when you lay out the symptoms this way. Not wearing a bra to treatment could be
chalked up to SI excess- she just isn’t that concerned about her appearance.
Then ask yourself what the grossest thing in the room is at each visit because she will be different every time.
I’m not seeing you mention body morphology or temperature. These are really important.
