A 37 year old female patient returned today to resume treatment (we did a few sessions back in January and
February before COVID). My Sa’Am questioning skills have changed considerably in the interim so I basically did
a new intake.
Main complaint: Cannot hold a pregnancy. Over ten years, she has had two miscarriages and 2 ectopic (‘tubal’)
pregnancies – all at the 6-8 week stage.
These are the 4-6 MAIN features of this case (there is really nothing else remarkable to note) . . .
Grossest Symptom: Lifelong extreme nausea tendency with just a slight odd smell, motion – even trying to text one
word while sitting in the passenger seat will bring on nausea. Oddly, during her 6-8 week pregnancies (all four of
them), she had NO morning sickness AT ALL.
VERY sensitive to light. Likes a room to be dim and even dark. MUST have sunglasses on on sunny bright days.
But I do not think she is particularly SanJiao excess . . . nothing remarkable about her eyes, could not care less if
pictures hang crooked on the wall, is fine if the house is a mess and there are other things to do, etc. And certainly
very pleasant but I would never say excessively polite.
I think this light sensitivity goes with the smell and motion sensitivity as all one thing. More on that later.
Periods are Heavy for 3-4 days and then mild for another 2-3 days, therefore also Long. But presently no pain.
(NOTE: back in January, I did actually do two Liver supplementing and one SI supplementing treatments – she did
have some menstrual pain back then).
Celiacs – tends to constipation and bloating but if avoids gluten, she is fine
Tends to be Cold. Not freezing, But it is a constant battle with husband who wants windows open and fans on and
she does not.
Very active and Loves sports – mainly loves team sports or running with a partner – but is highly non-competitive –
her softball team is terrible – she just loves to play with a group of people for the camaraderie but is not driven at
ALL by a competitive urge. Doesn’t like to run with her sister who is highly competitive – prefers to run with
people who are not.
has zero frustration in her life, her marriage, her work.
no skin issues, no respiratory issues (upper or lower), no sweat issues, sleeps just great. Body type is quite
unremarkable either way – no obesity, no thinness – just very average.
I really had to dig to assemble all of this into a Sa’Am picture. It suddenly struck me this was a Gall Bladder
deficiency candidate . . . . the common thread above is all about not pushing out enough . . . . ectopic tubal is an
incomplete movement out from the ovary through the tube to the uterus . . . the lack of competitiveness in sports . .
. the hypersensitivity to smells, light, even movement . . . seems to me to be possibly that she just is not pushing

constipation . . . and I can see how being even pregnant for a few weeks brought on some internal ‘pushing
outward’ which relieved her nausea disposition.
So then, from my ‘Kiiko past’, I palpated various channels and badaboom . . . . Gall Bladder and Pericardium
screamed at me!
So I did right GB+ treatment today. She got very relaxed. when I came back into the room 20 minutes, later she
reported a rush of intense nausea at the 6 or 7 minute mark of the treatment, that passed and gave way to two
minutes of pressure feeling in both hips that also passed and gave way to warmth in her right foot! She was very
relaxed when she left.
We shall see.

Thanks for the very interesting case Daniel. Her situation sounds like a great learning opportunity.
Where does she experience her nausea?

ectopic tubal is an incomplete movement out from the ovary through the tube to the uterus . . .
Yes, there needs to be enough movement in the right direction for an embryo not to get stuck. But then again, tubal
ectopics can happen in IVF cycles, where the embryo has no reason to go into the tube unless the uterus has
unsuitable environment.
I look forward to hearing what happens with her.

I am super interested to follow this case as well. having just been at Sa’am for a couple weeks, the things like
ectopic pregnancy = insufficient up and out of GB energy present a whole different perspective on understanding
physiology. Thanks for sharing your thinking.

Saam does invite us into considering physiology in a different way than we learned in TCM. What I’ve found
particularly interesting is when I get something to work with Saam, and then “translate” that to TCM.
So in this case we can see a kind of yang deficiency with embryo not being able to moved to uterus, or a blood
stasis/deficiency resulting an inhospitable uterine environment.
So often I found with TCM there was a pattern, but I could not quite grasp the dynamic underneath it. Saam helps
me with this. We should be able to take any Saam idea and see how it works from the TCM perspective as well.
