Clinic is Hard!!
Do you sometimes find yourself in front of the patient – you’ve gotten the whole story down and there you are, you
have to choose and needle within seconds (to keep clinic flowing) and you are teetering on that PC-GB continuum .
. . do I subdue the Tyson and encourage the librarian in this patient OR do I push the Tyson outward and leave the
librarian in this patient behind for now – and it really could go either way and yet it is clearly the PC-GB axis that
needs to be addressed in this patient!
So you make your choice and watch and wait.

Hmmm I asked about this on one of the recent crowdcasts regarding the LR/SJ conundrum. Toby’s response was to
choose another channel. Sometimes it can clarify the presentation depending on the patient’s response. Toby says
to have 3 choices to start. Was there not a clear 2nd choice in your PC/GB patient?
I have seen Toby walk us through this mental exercise in clinic as well.
But sometimes maybe both are deficient and need to be supplemented and can be toggled. Great example is the
SI/KD tonification in elderly.

This is typically the way you want to go. It is much riskier otherwise.

I think I wrote this on one of those clinical maelstrom days last week . . . you know, the pressure cooker clinic
situation where you are a bit behind schedule, there are two people in the waiting room and you gotta make a quick
decision?!
I am, however , finding I do have patients where all the information I can gather points to just one of the pairs (in
this case PC-GB) and no matter how much I ask and observe, I can’t seem to tease out any information pointing to
any of the other pairs!
