HI,
For a sprained ankle of the left side
most pain on the GB meridian 9+
swelling 7+
better with rest 8+
not purple
So there are 3 options here:
PC – because it is on the GB meridian and patient gets angry
LI – for the swelling
SI – for the pain – but it is better with rest so may not be the best choice
I chose to PC+ on right to open up the flow in the GB channel
I dont have feedback yet. Nothing miraculously cleared up on the table – but I did not expect it to because it is an
injury.
I am just checking my evaluation of which channel to use.
Should I have gone for LI + because if the swelling is reduced then the pain will be reduced?
Or would SI have been a good choice even though the pain is better with rest because creating good movement
with an injury is helpful regardless if it is better with rest?
Or was PC+ a good start?

I know Kristin will chime in here with more eloquence than I, but the way I evaluate it is just like we learn with
Sharon . Look at everything you see with the person rather than just the ankle
Do you see any GB excess signs? (for simplicity anger issues, red tip tongue, sleep issues )
Do you see any SP excess signs? (lack of focus, damp/damp signs, )
Do you see KD excess signs? (varicosities, , dark spots, oketsu, dark sublingual, symmetrical )
If any of those jump out it can lead you to a first treatment.
I would usually want to see some change on the table that first session, but sometimes it can take 24-48 hours for
change.

Hello,
I am assuming that the sprain was recent, so what I write speaks to acute injuries.
Because the issue was traumatic, blood stasis is part of the picture and is the initial cause of swelling. When the
tissues tear, the blood vessels break and bleed, even if you don t see bruising on the outside. With such injuries,
you often see the bruise percolate up at a later time.
I ve been meaning to write about better with rest being a potential SI indication because I think it can be
misleading in many situations such as this one. First of all, it is not a gem, it is a rock and not even Toby s rock but
a rock I found interesting. My thinking was that since SI excess is characterized by dynamic movement, it makes
some kind of sense that aggravation with too much movement AND better with rest could be seen SI excess in
some circumstances.
Clinically, this is not always useful especially for recent structural injuries which provide other explanations why
symptoms are better with rest. With a sprained ankle, activity involving being upright is going to make it worse
even if the ankle isn t weight bearing as it will increase swelling due to gravity.
Resting a sprained ankle, often means that the patient will have her ankle elevated which might reduce the swelling
and give relief. It might be better to say that SI excess is worse when tired which is different than better with rest.
But even then, that might be misleading. An area might get tired from a local positional strain, which is not the
kind of situation that supplementing K would be helpful for.
Those taking Sharon s GMP can refer to her discussion of diagnosing deficiency in relation to symptom response
to movement and tiredness.
Where it could be useful is when the symptoms are more systemic and not local or due to injury. Or if there is a
local injury in a patient with clear background of SI excess. What we want to see is real deficiency, a kind of
deficiency where the patient is not able to hold him or herself together and would benefit from the consolidating
aspect of K+.
I really apologize for introducing this misleading idea onto the forum without enough emphasis on how to use it.
With all of these ideas you have to go back to the gems from the intro class and not trust the extrapolations at
face value. In this case you want to ask, will this situation benefit from the consolidating function of Kidney+?

With ankle sprains, I want miraculous clearing. Whether using Saam or other methods, it has always been easy to
get some significant relief by the end of the treatment.

Thank-you for clarifying this. I can see better now that SI+ may be very good for certain injuries even is it is better
with rest. I did get tripped up by that rock but now I understand it better.

I agree that it is important to really consider what better with rest means in terms of deciding deficiency. To
determine deficiency it has to be worse when I am tired and better when I am well rested. Any injury is going to be
worse with use and needs time to heal so in this case, better with rest cannot mean deficiency.
