Saam post stroke patient

Apologies in advance – I don’t have this patient written up and only saw him relatively briefly this week but had a
few questions anyway.
50 year old male with left sided paralysis following a stroke 2 years ago. The stroke appears to have resulted from
a bit of arterial plaque coming loose and lodging in a blood vessel in his brain. As far as I can gather from him (his
memory isn’t always that clear) and his mother (in her 80’s and not quite completely with it) his surgeon explored
the idea of removing arterial plaque from his carotid artery but decided the risks outweighed the potential benefits.
So my first Sa’am thought with this treatment was SI+ – simply because he seems lacking in dynamic movement.
But I then wondered if a treatment that, as I understand it, powerfully moves things was appropriate given the
above. Reading here there’s plenty of suggestion that one be respectful of the power of Sa’am.
The second thing was the the patient mentioned impotence – so one might think KI+ but what seemed really odd,
given his situation, was that this was one of the first things the patient talked about and that made me think about
Ki excess. The patient’s marriage fell apart due to infidelity a few years before the stroke and he spent those few
years having a good time – which he was also keen to tell me about – more Ki excess
Lastly my reading here on the forum suggests that in a case like this the hypo-function is TGTITR (which it is) and
for that start with Ht+ – his limbs on the LHS are cold to the touch so that makes sense.
I’m at the beginning of the journey and have only used Sa’am with a few patients – I might not use it here but its
useful to begin trying to think these things through.

Thank you for your post and for Sa’aming Responsibly. It is a powerful system and you can go the wrong way
especially with a patient such as this.
You are on target with everything you wrote. There is the idea of caution if someone throws a clot and concern
about supplementing SI+. However look at your patient without the Western knowledge of the clot. Do you see
other clear blood statis signs? Varicose veins, lumps in oketsu area that are painful, purplish tongue, dark spots on
skin. If so you can think of KD excess.
But is interesting the impostence after the infidelity with his Jade stalk. I wonder if it is from Heart shock that he
got caught with his proverbial pants down and that led to the dissolving of his marriage? Or is the impotence

Since you see cold on the left paralyzed side. He cheated on his wife (lack of love for his wife could be in there), I
would think that from what you presented a supplement HT+ would be the safest. Just make sure there is no clear
heat signs, i.e. he is overheated, rapid heart rate, dryness in body. You didn’t give his morphology but you would
want to be cautious if he is super skinny and dry.

Hi George – yes that’s very helpful. I always try to not let western medical diagnoses guide my thinking but
perhaps have got caught here – I find if I do I usually confuse myself – it’s like trying to mix water and oil.
Interesting thoughts on the impotence – I’d not thought in those terms. And I’d not noted any blood stasis type
signs. Seeing him again this week and will look for signs of heat and if they’re not there may do HT+ with him.
