Psoriasis and tonify Bl

In a psoriasis case, where the body speeds up the life cycle of the skin cells that could be considered
hyperfunctioning and tonify Bl could be appropriate. But if there are some cold signs such as period pain that is
better with a hot water bottle and that the psoriasis burns more with cold air, would that negate tonify Bl?
And if yes, is there a specific way to see psoriasis through Saam eyes?

That understanding of psoriasis could have some clinical weight for H excess.
The actual presentation of the psoriasis will have much much more clinical weight: how red, how purple, how
itchy, how dry/moist. A hot skin condition will be more likely to be relieved by contact with coldness. Does it make
a difference if the cold air is hot or dry?

Toby hasn t spoken about psoriasis specifically to my knowledge. In my experience, psoriasis can vary in its
presentation. Keep going back to Sometimes channel, sometimes quality.
My experience from other styles of acupuncture and herbal practice is that for chronic skin conditions in general it
is really helpful to consider what in the system is preventing normal nourishment and circulation in the skin.

I could use some more specific help on the psoriasis case
23 year old woman. A little overweight but not extreme. pale face with little red zits all over – mostly on stomach
channel.
M/C: psoriasis of the scalp and right ear. Bladder and Gb meridians on the

underneath is red.
The scalp, itches, burns, and is painful – usually alternating between
itchy and painful.
b/w coconut oil – but relief is short lived
w/w going out in the cold wind
w/w stress
w/w getting a cold – sniffles, runny nose, chilled (wind cold)
feels hot in general, hotter at night
but when touch skin her feet, legs, knees, back, and arms are cold
her stomach and hands are warm
stools: 4-5 times a day, stool is formed
Menstruation: constipated before period, diarrhea when period arrives
nauseous before period
PMS – irritable and breast pain before period
cramping with period, clots
sweat more with period
sweating: sweats a lot in the heat
has asthma: w/w cold and exertion b/w hot shower
feels tired in the morning, but when get up and start doing things gets
energy
pale pink tongue, sides pale, red tip
SJ strong LRST +8 bright eyes +7
LU damp on inside dry on outside +7
Ht feels hot +6 hyperfunctioning of prod. Of skin cells +8 thirsty for cold water +6 stools after every meal +6 dry
skin on Bl mer on foot +7 dry itchy painful burning skin on Bl mer. on scalp
Kid scalp is itchy +7 scalp is painful +8 movement helps energy level +7 period blood is darker red and have clots
feels hot +7
SI not symmetrical +7 stools after every meal +7
Bl hot water helps period pain +7 dry skin on Bl mer on foot +7 dry itchy painful burning skin on Bl mer. on scalp
arms, legs, knees, feet and back are cold to the touch +7 asthma worse with cold +7
GB dry itchy painful burning skin on GB mer. on scalp +9 gets frustrated and irritable at times and esp before
period +5
PC dry itchy painful burning skin on GB mer. on scalp +9
Treatments
First: PC – no change
2. St – skin comes off more easily – dont have to pick it off so less bleeding
3 SI – less burning and less itching
4. SI again – also less burning and itching but then caught a cold and symptoms flared up back to how it was when
she first came
5. Ht – no change
I would love advice and suggestions on how to continue.

I m not sure what LRST is but am interested because it is 8+!
Red tip of tongue, when it is clearly redder than the tongue body, adds weight to the GB excess.
What is the rest of her skin like, especially in terms of moisture? How does she feel about moisturizing- people
with damp on the surface/blocked surface will feel suffocated by heavy moisturizers If she has generally moist skin
and/or fog, I would consider LI+. The frequency of the bowel movements make me curious about this too. Are
these frequent bms really normal- not loose or urgent?
H+ didn t change any of her symptoms at all? Any adverse reactions? It sounds like she has a good amount of
internal heat, the psoriasis is on the UB channel, is red and burning and is worsened by getting a cold. This makes
me curious about UB+. Wintertime makes me cautious about using UB+, assuming your area gets some cold
weather.
The mixed hot and cold signs add weight to SI and K excesses. Not symmetrical 7+ along with the mixed hot and
cold makes me curious about K+ but maybe after trying some other things first, including possibly ST+ again.
Let us know what you do and how she responds.

LRST = Left rib side tension
Her skin is not moist. She has patches of dry skin – where there is minor psoriasis. The only things that helps those
dry patches is moisturizing.
It is hard to imagine that LI + could help when her dry patches and scalp are relieved by moisturizing. Am I not
seeing something correctly?

H+ did not change her symptoms. I was also surprised.
I would also like to try UB+ but I am nervous to do so because her symptoms are worse in cold wind and also get
worse when she gets a cold.
The same is true for LIv +. The liver channel does go up to the head and it can nourish the blood and is for hot
above and cold below – which is her presentation. I am apprehensive to do LIv + though because her period pains
get better with a hot water bottle which indicates cold blood stasis in the uterus.
It is such a mixed hot/cold presentation that I am finding it difficult to move in either direction.

It sounds like moisturizing is soothing, but not really healing.The psoriasis is dry and flaky but it is also raw, not
nourished and not healing. My line of questioning was to get a better sense of her overall surface. We know she
sweats extra in the heat and before her period so that is a sign of damp at the surface. If the rest of the skin was
moist, or if she is overall sweaty, or if she has fog, then it would make the case of a damp exterior. In that case, the
raw patch of psoriasis could be seen as a rotten area in a swamp.
This line of questioning is also inspired by a recent experience treating my own dry, rough, itchy skin successfully
with LI+. Damp dry interior exterior
In the absence of damp on the exterior, I would consider ST+, Liver+ and UB+. Of the cooling treatments, her
response to Liv+ will give you a better idea of how tolerant she is to cooling. Heat will relieve non-cold related
cramps as well as cold related cramps as heat moves qi and blood. Given her variety of significant and clear heat
related signs, cramps better with heat isn t enough to shy away from cooling her down.

Damp dry interior exterior
Fog is a soft, fluidy layer in the subcutaneous tissue. To evaluate the subcutaneous rest a relaxed hand lightly
on the skin. If there is fog, you will feel it floating on a cushion of fluid. It doesn t feel like healthy fat.
Compare that to other patients who are dry and desiccated or to other qualities of subcutaneous tissue. It should
have a nice firm integrity. Fog is pretty common, even in thin patients. Sometimes it is quite dramatic.
Here is another case of where drying the exterior helped a patient with very dry flaky skin and a slow healing
wound. This case Lung+ was used instead of LI+. In hindsight LI+ might have worked too since she clearly had a
damp interior as well. Slow Healing Wound Healed in 1 Treatment
