Hi
I have a patient that claims she usally has cold feet and hands during daytime, But during the night the feet gets
hot. Im obvously think of san jiao excess, because of the cold hands/feet. But i cant get my head around the hot feet
at night?? Does anyone know??

Only hot feet at night is one that I don’t know if I’ve come across especially if she has cold extremities during the
day. The rest of her is not hot just her feet? Interesting!
I am not sure how to explain the hot feet without knowing the rest of her presentation.
For SJ excess Is she sensitive during the day to temperatures? That is more what I think of in the SJ excess
checkbox. Does she have KD excess signs? SI+ supplementation can work well for cold extremities if that pattern
fits.
I would suggest doing your Sa’am diagnosis and consider the grossest thing in the room. (GTITR). How would
you treat her without knowing about this cold/hot dynamic?
Anyone else have thoughts on this?

I would first think about the conformations that are each mixed heat/cold: SJ/LV, SI/KD.

Hey folks,
I was under the impression that SJ xs could present w/ cold feet ( Not hands)) as qi is upward. Hence,
supplementing Lv would warm feet and cool upper body. Conversely, supplementing SJ would do the opposite.
Any disputes or confirmations on this please.

SJ excess heat above/ cold below. – red cheeks, cold feet are the quintessential symptoms.

I would not pair HT/LR on a first treatment. Pick one channel so you can see clearly what is going on for the
patient. We do not have a complete diagnosis for this case so cannot say more.
