47 Year old woman
Chief complaint: Overwhelmed, uptight, tense, anxious, crying easily, getting angry.
Ever since she has gone back to full time teaching after teaching on zoom from home she is overwhelmed and not
herself. She is also planning a big weekend party for her son’s Bar Mitzvah this weekend. She is not sleeping well
and is anxious and uptight.
Second complaint: Pain under both breasts that also goes around to her back – like a circle that starts under her
breasts. Her whole upper back down until Bl 19. Its hard and tight and painful. On her shoulders its particularly
painful on the SJ and GB meridians.
The pain is worse when its cold outside or when the AC is on. Better with warmth. It is also worse in times of
stress.
Appearance and demeanor
A bit overweight, symmetrical, soft flesh
Wide at the hips and thighs
Kind eyes
Eyes not bright and not particularly dull
Dressed casually, wore nice earrings and rings, nails done nicely
Easy to reach. Cried easily a couple of times during the intake.
Lifestyle
Married, a mother of 4. Youngest is 9 years old.
A teacher of HS students. Has been teaching for over 20 years and likes it.
Enjoyed teaching from home during corona.
Is having a hard time transitioning back to full time teaching in school.
Likes to be busy. Says she is usually optimistic and happy.

Not dry.

Waking up at 4:00 for the past 2 months. Can sometimes fall back asleep after an hour and sometimes not.
Emotions
Has been getting angry lately
Irritable, uptight, stressed, anxiety
Low energy, tired
Says lacks her usual happiness and excitement
Her mom got ovarian cancer when she was 10. She was sick for many years and died when she was 20. She had
her own ovaries removed 8 years ago.
She did not seem to be over focused on that trauma but it is a trauma none-the less.
Body:
Some light varicosities on thighs but not pronounced
Medial heel fleshy
Thenar eminence – not sig.
Line from sternum to umbilicus is more toward the soft and warm side
Upper back and shoulders hard, tight, and painful
Hurts when press under right ribs
Pain on the liver, SJ, GB, and Bl meridians.
Tongue
Deep crack down the middle

Main ideas: PC, Liv, Ht
The first treatment I did was PC+ (R)
To calm the anger, help her sleep, calm the rising GB energy and give her some grounding and rooting.
For the second treatment I am debating between HT and Liv
For Liv – she seems like she could use a shield and all of this anxiety came when she had to leave her house and go
back to teaching at school which makes me feel that her shield was snatched away suddenly and she could use a
shield to go back out there to the world and deal with busy post corona life.
But – she is a teacher and likes standing in front of crowds and teaching so that means that she already has a shield
doesn’t it?
Also – her pain is worse with cold -which makes me hesitate to use Liv+.
Ht+ may be good because her pain is worse with cold and she says that she is missing her spark.
But I thought HT+ may be too stimulating for someone who is overwhelmed and overstimulated with so many
things to get done. It is also so hot outside here now and her pulse is on the faster side.

How did she respond to this treatment? Reading through your case supplementing PC+ would have likely been my
first go to. (red dots on tongue tip pushed it there for me).
When I have unclear cases I do my best to choose the channel and their response helps dictate what direction to go
in. So what comes next would depend on the first treatment.
Assuming a some positive response to the PC+ I would consider a pair of LR/PC+ to help subdue the anger and
overwhelm.
She also has pain along the SJ/GB meridians.
Just because someone is cold don’t jump to supplementing HT+ and in this case it does not feel like she needs
more outward loving/fire – she is doing all this planning for her son’s Bar Mitzvah

HI, She seemed to settle and felt good after but I will only know for sure tomorrow morning when she comes.
I feel like LIv+ would probably be good for her but I am scared it will aggravate the pain since cold makes the pain
worse and Liv+ is cool.
What do you think about that?

I think LR+ is most appropriate for her. Do not get lost in the details of hot/cold on channels. Interestingly I’ve
observed temperature is the most common characteristic/symptom for people to be incorrectly tripped up on.
Remember we need to look at the entire picture. Liver is only slightly cooling. It is a cool mist and shielding.
Just because a channel has a tendency towards a temperature do not let that deter you from treatment if the rest of
the picture fits. For this patient LR+ fits well as a top 3 channel consideration for sure.
With that said I have supplemented BL+ on several skinny freezing cold women with great success. (they were dry,
slight rapid or rapid heartbeat). If we give so much weight to the temperature we can miss providing the correct
treatment.
