Back to Square One

I had a patient coming in with limited ROM and pain in the shoulder. MRI showed bursitis, delamination of
supraspinatus tendon. Using SAAM, I treated SI+, LI+, P+ and getting major progress of 75% improvement. Then
I did UB+ which the day of the treatment, he felt great and noticed increased improvement but the following day,
he lost all gains and we were back to square one. I read that as a possible adverse reaction so I did H+ to reverse
the treatment. H+ didn’t seem to do much for him. Since then, I did LI+/ SI+ combo, and LU+. He has no strength
in the arm and can’t lift. I am so bummed that we’ve lost all gains. Any thoughts as to what went wrong and how to
proceed?

HI Maria
Hard to know why the patient regressed without more details about the patient.

How far apart were these?

Odd that he had increased ROM and next day he lost it. Did he overdo it when his shoulder felt good. We’ve all
seen that where a patient does too much and then the benefits are lost. Feeling good and increased ROM after a
treatment I would not think it is an adverse reaction. Something doesn’t add up here.

How many days from the BL+ treatment was it? And I am not convinced BL+ was an adverse reaction.

When you first supplemented SI/LI were they at the same time or separate? I have noticed what works as a single
may not work as a pair and vice versa.
Also it seems you supplemented PC+ originally but not this time. Do you see some GB excess still?

I have not had consistent positive outcomes with ‘frozen shoulder’ with Sa’am… Have you done the
deconstructing class using channels only?
You have not mentioned the LR/SJ pair – SJ is certainly a major shoulder channel.

<How far apart were these?> I had him coming in 2x/wk so the tx were spaced a day or two apart
He was showing off to his wife how great the shoulder was after the treatment. I don’t think he overworked it.
Besides, it shouldn’t go from 75-80% back to 0% even if he over did it. There should still be some residual
improvement, right?
< And I am not convinced BL+ was an adverse reaction.> Me too, esp. since H+ didn’t have any impact. But this
leaves me in a quandary as he does have knee pain at UB 39, 38. Should I try UB+again? He is very talkative,
friendly guy and feels warm easily.
< Do you see some GB excess still?> Yes. P+ is certainly on the table. But since the regression, he decided to drop
back to 1 visit per week.
<Have you done the deconstructing class using channels only?> No. Not yet.
<LR/SJ pair – SJ is certainly a major shoulder channel.> Thanks for the reminder! I meant to try LV+ but got so
sidetracked with the regressive stuff.
<I have not had consistent positive outcomes with ‘frozen shoulder’ with Sa’am> I am curious what other issues
have you not had consistent results with Sa’am

How long was it improved? Something is missing here that he had a complete reversal without doing anything.

You did it once. Do you see any clear dryness signs?
If so I might consider BL+ again since it appeared to work. (was BL+ the biggest gain?)
Or consider going back to the channel that gave the most improvement.

I recommend it.

He was dramatically improving over a course of 5 treatments. He felt great with 80% increased ROM after that
treatment into evening. Then he noticed pain in the shoulder that night and all gains were lost where he could
barely move his arm.
I am 4 treatments in after the regression and the gains are now painfully slow
