Strain/Sprain of L Thenar Eminence and Tendons on LU Channel

60 yr old F pt came in with a workers comp case. While lifting heavy items at work pt reported a strong pulling
sensation along L hand and forearm. Hours later her L hand and arm were throbbing in pain. She reports being
unable to lift lightweight items like spiral bound notebooks, twist doorknobs, and is sensitive to the lightest
pressure and movement. The incident occurred 2 days ago.
Today was the first day I saw her. Her pain was on the left LU channel at the attachment of the thenar eminence to
the first metacarpal and tendon/muscle pain from LU 9 to LU 5. Her L hand was visibly swollen compared to her R
hand. Pain alternating dull to throbbing and rated at 8/10.
I put in points for SI+ (To treat blood stasis and swelling d/t trauma) on her right side and left her for 20 mins.
When I came back she reported feeling warm all over and like her body was pulsing.
I asked her to gently move her L wrist and hand. She reported no pain. I saw that her swelling was gone. She
propped herself up off the table with her L hand and lifted her purse with it as well. No pain! Too cool!

Great clinical response and a good reminder of my teacher s clinical heuristic – sometimes channel, sometimes
quality .
