How does your Sa’Am clinic work?

Between July 2020 and now, I went from Saam 0 to Saam 9 in terms of clinic usage. Barring the occasional patient
whose intake needs digging and sorting and parsing, in general the sessions now need less time. For many years,
I’ve run two rooms, with 30min between the first 2 and last 2 patients, and 45min between all the others. This
much time has in the past accommodated the acupuncture and making herbal formulas, but now I have more down
time.
I would love to hear from you more seasoned Saam practitioners out there about how you’re scheduling people:
how long between each patient? how many rooms do you run? how many patients can you see in a shift/day before
it feels like your brain can’t Saam anymore?
Also, Toby has repeatedly said that when he treats a new patient, he’ll want to see them 2x/week. Does anyone
know how he accommodates this in his schedule? Does he reserve a day to fit people in as needed?? Or are new
patients automatically scheduled with a 3-day followup?? I would like to have this kind of follow-up, but if my
schedule is full already, it’s hard to fit people in.

I too have found that I need less time with people when doing Saam. At the moment due to the infection rates here
in MO I’m only seeing one person at a time, but usually I run two rooms. As to time between people, I know that
with certain people it goes pretty quickly and others I’ll need a little more time. I also need time to do herbs, so that
gets to be a significant constraint.
If I was not doing herbs I could probably run three rooms and not break a sweat.
Also, with Saam I’ve found I use less herbs, mostly because I want to get in one or two treatments first and see
how it goes with the acupuncture. While I’m an herbalist, I’d say that part of my practice has slide a bit with Saam.

I feel like Sa’am has added years back onto my life. It took a while at first to go from a Kiiko style with 3
treatments to 4 needles and done in 30 mins. (I remember one of my first long time patients getting Sa’am she said
“That’s it?” I said “How do you feel”. She said “I feel great”. I said “see this style is efficient!”)
I don’t push myself. A busy day for me is 15 in 7 hours, more typically about 10 in 5-6 hours. I find my ability to
give wanes as the day goes on. I have 3 rooms and schedule each room on the 90 minutes. I spend from 10-20
minutes with each patient at the start checking in and connecting. The amount of time I spend connecting and
tuning into patients has dramatically increased with Sa’am. Before my acupuncture felt more mechanical, now with
Sa’am I am in a different space and it feels good/deeper/connecting. (Which is also tiring if I don’t protect myself.)
After check in I needle and let them cook. I give them a bell to ring if anything comes up (gets cold, gets hot, gets
uncomfortable, gets bored). I rarely go back in to manipulate the needles. But sometimes if I want to do 2 sides I
may add in one side and then go back in after 10 minutes to check on progress before adding side #2. For difficult
cases or risky treatments I may stay with them in the first 10 minutes after needling.
I usually leave them for 30-40 mins or so, but instruct them that if they feel deeply relaxed for a good while and
then ‘wake up’ and feel they are done to ring the bell as the needles have done their job.
With that time in between often I am writing/filling herbal formulas or eating. I have not gotten to the Toby stage
of meditating between patients.
I had an office manager who did all my check outs/calls etc but with Covid she is only working remotely for me
now. Therefore most of my patient’s credit cards are on file with my EHR and I run their card at the end of the
session or do it at the end of the day.
I used to use sheets for everyone but with Covid I am not just using table paper and a wipe down. It seems a lot
quicker than linens.
I know some people on this list did tighten up their schedule with the saved time because of Sa’am and they can
give you a more interesting answer than I provided.

Hi Sa’amers! I am relatively new to Sa’am, but am using the treatments for 99% of my clients. I am trying to
understand the time that Toby recommends NOT using Sa’am. I see several patients weekly, and am wondering if I
can be overusing this powerful method? When I took Toby’s class in June he mentioned doing 3 initial treatments,
then give herbs for remaining symptoms and wait one month to do another 3 visits. What if my patients want to be
treated during that month? Are other Sa’amers using other methods during that time? Thanks for your input! Kiki

half hour, and see how it feels. Good idea with the bell. How often do patients ring it?

San Jiao excess patients hardly ever ring it. Or if they do it is so quiet I cannot hear it.
Overall not often, but it gives a lot of patients comfort that they aren’t stuck if they need help.

This is what I sometimes do but it’s not a rigid guideline.
